The SF 9 or Learner's Progress Report Card is a document used by schools in the Philippines to track the academic progress of students from kindergarten to grade 6. The SF 9 is divided into four quarters, and each quarter includes the following information:
  • Grades in the different learning areas
  • Attendance
  • Behavior
  • Remarks from the teacher
  • The SF9 also includes a section for the parent or guardian to provide feedback on their child's progress.
The Department of Education (DepEd) has made available an editable copy of the SF 9 for free download. This allows teachers to customize the report card to meet the specific needs of their school and students.

To download the editable SF 9, visit the DepEd website and search for "SF 9." You will find a link to the document, which you can then save to your computer.

Once you have downloaded the document, you can open it in a word processing program and make the necessary changes. Be sure to include the school name, school year, and student's name and LRN.

You can also add additional information to the report card, such as the student's goals for the next quarter or their strengths and weaknesses in specific areas.

Once you have finished making changes, save the document and print it out. You can then distribute the report cards to parents and guardians.
