What are COT Forms?

Classroom Observation Tools (COT) are forms used by teachers and administrators to observe and evaluate classroom practices. These forms are part of the Results-Based Performance Management System (RPMS) and are used to assess the performance of teachers from Teacher I to Master Teacher IV.

How to Download COT Forms?

DepEd has made these COT forms available for download online. The forms are updated every school year to reflect any changes in the curriculum or teaching practices. For the school year 2023-2024, the updated COT forms for Proficient Teachers (Teacher I to Teacher III) and Highly Proficient Teachers (Master Teacher I to Master Teacher IV) can be downloaded from the DepEd Click Facebook page.

In addition to the COT forms, DepEd also provides a variety of other resources for teachers, including the RPMS tools for School Year 2021-2022, FAQs on the Alternative Classroom Observations for RPMS SY 2021-2022, and the RPMS Tool for Teacher I-III Full-Time Teacher-Broadcasters in the Time of COVID-19 for SY 2021-20221.
