Checking of DepEd School Forms is waving so that teachers are busy as bees these days completing students' permanent academic records. One of these permanent records is SF10 which is formerly known as Form 137. 

Plenty of sites in the internet offer downloadable DepEd forms, however, none provides teachers a document that is in word format. Most SF10 are either in excel or PDF format. So to give our beloved educators who find it more comfortable using the Microsoft Word, we would like to share with you SF10-JHS in word format.
